SBH is seeking a qualified individual who will work within the substance use program team of providers that works directly with the individuals seeking and/or receiving treatment for substance use disorders. The individual in this position provides intake services for SUD which includes completing the Government Results and Modernization Act (GPRA) assessment. The assessment must be completed for all identified individuals receiving services either from a State Opioid Response (SOR) funded provider or receiving services funded through SOR at intake, every 6 months while in services and after services have been completed. This assessment can be completed either via in person, over the phone or via telehealth. This position requires excellent communication skills, interpersonal skills and the ability to connect with the individual receiving services. This is a grant funded position and depends on grant funding annually to maintain this position. Should grant funding be eliminated, this position would also be eliminated.
Responsibilities
- Provide excellent customer service skills by phone, computer or person.
- Use technology in an efficient and effective manner understanding email, telehealth systems and phone systems.
- Complete assessments in a timely manner submitting all assessments within 24 hours of completion.
- Use the state data collection website for all GPRA assessments for both the assessment process and submission of the assessments.
Qualifications
- High School Diploma or equivalent required. Bachelor’s degree preferred.
- Must be able to use technology, computer, phone and interoffice communication systems. Must be able to use communication platforms, google docs, sheets and gmail. Must be proficient in the use of Microsoft Word and Excel.
- Must meet all criminal background and drug screening requirements.
- Valid driver’s license and safe driving record, required.
