This non-exempt (FLSA) position’s incumbent provides discharge planning and case coordination services to individuals identified with Serious Mental Illness (SMI) and who are either in restoration services, NGRI or at a high enough risk to receive intensive case management

Responsibilities

This position involves assessment of risk, coordination of assessments, preparation of
service plans, coordinating and accessing service delivery programs, necessary support
services, pre-admission and pre-discharge planning and casework documentation. The
objective is for the client to maintain ongoing stability and to function at his/her highest
level of productivity in the community. Coordination of services include, but are not
limited to, coordination of services, participating in treatment team meetings in state
facilities, attending discharge planning meetings at SBH, participating in forensic
treatment planning and community support, advocating, linking, coordinating,
monitoring, problem-solving and coordination of transportation. These services also
include the coordination/provision of restoration services, communication with the court
system in a professional and timely manner and collaboration with all state systems
involved with the individual.

Qualifications

Bachelor’s degree in Human Services and QMHP-A Required. Provision for the mentally disabled population with a major area of study encompassing evaluation, assessment, treatment/service planning and casework documentation as well as utilization of available community resources for the mentally disabled. Must meet all criminal background and drug screening requirements. Valid driver’s license and safe driving record, required.
